To become a MANGO Qualifier please fill out the Welder Qualifier Questionnaire and submit it to Chad Owens. Qualifier shall be an AWS CWI and shall fill out a separate from for each individual to be considered. Qualifiers are to use the four part form: MANGO API 1104 Welder Qualification “Destructive Test Report”. These forms are available for download on the Forms for Qualifying Welders Section of the web site. Qualifier shall supply the welder with two copies of the destructive report and shall keep one copy on file. Qualifier shall submit one copy (white) of the form along with a completed internet permission form to:
